St. Johns County has grown quickly and its medical infrastructure has not entirely kept up, particularly for specialist care. For a great deal of the county, the default assumption is still that anything interventional means driving into Jacksonville.

For patients in St. Johns, Julington Creek, Fruit Cove and much of St. Augustine, Nocatee is closer — usually via CR-210 or US-1 — and the visit is shorter at the other end because there is no hospital campus to navigate.

The county also has a substantial older population, which brings the conditions that go with it: spinal stenosis, facet arthritis, knee and hip osteoarthritis, and peripheral neuropathy. These are precisely the problems where a careful diagnosis changes the treatment, and where "manage it with medication" is too often where the conversation stops.

Patients from across St. Johns County most often arrive with stenosis, arthritis or neuropathy that has been managed rather than investigated. A proper diagnostic work-up frequently finds a specific, treatable source that was never identified.
