Three problems, one complaint

People point to three different places when they say "hip". Pain in the groin is usually the hip joint itself. Pain on the outside over the bony prominence is usually gluteal tendinopathy or trochanteric bursitis. Pain in the buttock is often the lower back or the sacroiliac joint. Getting this wrong is why so many "hip injections" fail.

Ultrasound settles most of it in a single visit, and the treatment that follows is specific to the structure rather than to the region.

    Locate it precisely

    Groin, side or buttock. That alone reorganises the differential before anything else happens.

    Ultrasound the soft tissue

    Gluteal tendons and the trochanteric bursa are superficial and image beautifully — tendinopathy is visible directly.

  3. 03

    Diagnostic injection where useful

    A guided injection into the hip joint that relieves the pain confirms the joint is the source. One that does not is equally informative.

    Treat the confirmed structure

    Joint, tendon or spine — each has its own set of options.

Questions we get asked

I had a hip injection elsewhere and it did nothing. Why?
Two common reasons. Either the injection did not reach the joint — the hip is deep and unguided injections are unreliable — or the joint was not the source of the pain in the first place. Both are worth establishing before repeating it.
Can you help hip pain after a replacement?
Often. Pain after hip replacement can come from the surrounding tendons, the bursa or the lower back rather than the implant itself, and those are treatable.
