Your first visit, start to finish.
Most people arrive having already been somewhere else. The first appointment is about establishing what is actually generating your pain — so bring everything that helps answer that.
Before you come in
What to bring
- Photo ID and your insurance card.
- Current and previous medical records relating to the pain.
- A list of your current medications, including doses.
- Any imaging you have had — MRI, CT or X-ray. The report alone is useful; the actual images on a disc or portal are better.
- Notes from any prior treatment: physical therapy, injections, surgery, chiropractic.
- A referral, if your insurance plan requires one. Call us if you are not sure.

How it works
What happens when you get here.

History and examination
Where the pain is, what makes it worse, what has already been tried, and how it is affecting what you can do. Then a physical examination focused on reproducing and localising it.

Imaging reviewed with you
Dr. Loch reads your MRI or X-ray on the screen with you. Where a dynamic look helps, ultrasound lets us watch the structure move rather than judge it from a still image.

A working diagnosis
You leave knowing what we believe is generating the pain and how confident we are. If two explanations remain, we will say so and explain how a diagnostic block would separate them.

A plan
Treatment options in order of how invasive they are, what each involves, and what we would expect from it. Many procedures can be scheduled shortly afterwards — the procedure suite is in this office.

I have been to many Pain Management Doctors and have finally found Dr Loch. I never feel rushed or uncomfortable. Dr. Loch and his amazing staff are extremely friendly and supportive. Instead of treating me as a patient, I am treated with compassion.
Over too many years I have seen too pain management guys. This one is different. First, he caught something on an MRI that only one other health professional had caught, including those reading the MRI, a cyst at the base of my spine pushing again the nerve. Second, his (their) treatment of it was first class. He took his time, was gentle. Concerned about my pain level throughout the process. Talked. Was funny, or at least amused by my jokes, a nervous habit and caring. Great experience!
My first visit with the "Ponte Vedra Spine & Pain Center" was "exceptional". The staff were very friendly & welcoming as well as professional. My meeting with Dr Loch was equally "exceptional." He carefully reviewed the MRIs & and evaluations of previous physicians, listened attentively while I reported the origins of my back problem through my current condition. He then fully explained the available procedures to treat my back pain & was very frank about how effective each procedure could be in alleviating my pain. Our visit concluded with a thorough plan to address my condition until our next meeting; Dr Loch prescribed medication to manage my pain, and set up a follow up appointment. The staff is amazing and always friendly. After just a couple treatments, my shoulder is feeling better. Definitely recommend to anyone with back or joint pain.
