About us
Built around finding the cause.
Ponte Vedra Spine and Pain Center opened to give Nocatee and St. Johns County access to advanced pain treatment without a drive to a hospital system.
Our mission
Our core belief is taking a comprehensive approach to pain treatment. We believe that only through trying to correct the root cause of the pain can patients achieve long-lasting relief and improvement in function.
That belief shapes how the practice is set up. Diagnostic ultrasound, live X-ray guidance and the procedure suite are all in the office, so the visit where we work out what is wrong and the visit where we treat it are not separated by weeks and a referral. Dr. Loch reads your imaging with you, in the room, on the screen.
It also shapes what we do not do. We are not a practice that manages pain indefinitely with escalating prescriptions. The aim is a specific diagnosis, a treatment matched to it, and a plan for what happens after the pain lets up.
What that means in practice
What you can expect here
- Your imaging is reviewed with you on screen, not summarised at you.
- Injections are placed under live X-ray or ultrasound guidance, not by landmark.
- Where the source of pain is ambiguous, a diagnostic block is used to confirm it before treating.
- Minimally invasive options are worked through before anything more involved is recommended.
- You leave with a plan for rebuilding function, not only with a procedure date.
- One physician sees you every visit. You are not passed between providers.

In their words
What patients say
Great experience from start to finish. Staff is super friendly and knowledgeable. Exceptional customer service. Dr. Loch is awesome at explaining everything and painting the full picture so I can make decisions based on my needs. Highly recommended. 11/10
I go to the top position and must’ve been but lots alike so that sits at the very top. I refer him to anybody. I know that’s in pain because I know he has the knowledge and skills to help him.
I have been to many Pain Management Doctors and have finally found Dr Loch. I never feel rushed or uncomfortable. Dr. Loch and his amazing staff are extremely friendly and supportive. Instead of treating me as a patient, I am treated with compassion.
Over too many years I have seen too pain management guys. This one is different. First, he caught something on an MRI that only one other health professional had caught, including those reading the MRI, a cyst at the base of my spine pushing again the nerve. Second, his (their) treatment of it was first class. He took his time, was gentle. Concerned about my pain level throughout the process. Talked. Was funny, or at least amused by my jokes, a nervous habit and caring. Great experience!
My first visit with the "Ponte Vedra Spine & Pain Center" was "exceptional". The staff were very friendly & welcoming as well as professional. My meeting with Dr Loch was equally "exceptional." He carefully reviewed the MRIs & and evaluations of previous physicians, listened attentively while I reported the origins of my back problem through my current condition. He then fully explained the available procedures to treat my back pain & was very frank about how effective each procedure could be in alleviating my pain. Our visit concluded with a thorough plan to address my condition until our next meeting; Dr Loch prescribed medication to manage my pain, and set up a follow up appointment. I could not be more impressed with the Spine & Pain Center and happier with my decision to seek treatment with Dr. Loch & staff. J. B. Egan CAPT. USCR Ret.
The staff is amazing and always friendly. After just a couple treatments, my shoulder is feeling better. Definitely recommend to anyone with back or joint pain.
